Section 1: Essential Documents & Information

Before your appointment, you will need to gather and submit these items to your chosen clinic. Have them ready in a digital format (e.g., PDF).

Document / Information Notes Status
Summary of Care Record (SCR)
This is a summary of your medical history from your GP. It is essential and non-negotiable.
Photographic ID
A clear photo or scan of your Passport or Driver's Licence.
List of Current Medications
All prescription and over-the-counter medicines you currently take.
